PLACES (Population Level Analysis and Community EStimates) is a collaboration between the CDC, the Robert Wood Johnson Foundation, and the CDC Foundation. It provides model-based, population-level health data for small areas across the United States, including counties, places (incorporated and census-designated), census tracts, and ZIP Code Tabulation Areas (ZCTAs). It allows communities to better understand the burden and geographic distribution of health-related outcomes in their areas, which is crucial for a detailed community health assessment.
